A few years ago we had some stuff hosted at the premier facility in SLC. Redundant redundancies galore. They had a tech working on the fire suppression system and he over-charged it. I'm not sure of the details about what happened but there was a very big bang. Supposedly a couch in the NOC ended up going through a window. Nobody hurt and as a matter of fact we didn't even go down. It can happen anywhere and when expensive *feces* goes boom it's always a bit of luck if people don't get hurt.
