A cap is an international appearance.  You have to play, not just be named to the team.  Once you step foot on the pitch you get the cap.  Derives from some goofy English tradition of giving players a hat for the appearance. 

Applies to rugby and probably other sports too.  Speaking of rugby, the World Cup starts today.  Matches are shown on Setanta.  I'll have to start another thread.
